    Syndromic Diagnosis of Malaria in Rural Sierra Leone and Proposed Additions to the National Integrated Management of Childhood Illness Guidelines for Fever

    Many countries in Africa, including Sierra Leone, have adopted artemisinin-based combination therapy as first-line therapy for treatment of patients with malaria. Because laboratory testing is often unavailable in rural areas, the cost-benefit and viability of this approach may depend on accurately diagnosing malaria by using clinical criteria. We assessed the accuracy of syndromic diagnosis for malaria in three peripheral health units in rural Sierra Leone and determined factors that were associated with an accurate malaria diagnosis. Of 175 children diagnosed with malaria on syndromic grounds, 143 (82%) were confirmed by the Paracheck-Pf test. In a multivariate analysis, splenomegaly (P = 0.04) was the only clinical sign significantly associated with laboratory-confirmed malaria, and sleeping under a bed net was protective (P = 0.05). Our findings show that clinical malaria is diagnosed relatively accurately in rural Sierra Leone. Incorporating bed net use and splenomegaly into the national Integrated Management of Childhood Illness guidelines for evaluation of fever may further enhance diagnostic accuracy for malaria

    Ground Water in the Kentucky River Basin

    Most private wells in the Kentucky River Basin are in unconfined or semi-confined bedrock aquifers. Within these aquifers, high-yield zones are irregularly distributed. The most productive wells are drilled into fractured bedrock and alluvium along the Kentucky River floodplain. The data indicate that ground water acts as a buffer to peak and low flows in Kentucky River Basin streams. At current withdrawal rates, ground-water usage does not seem to have an adverse impact on the Kentucky River. Privately owned ground-water sources supply approximately 135,000 people living in the basin-approximately 19 percent of the total population and 36 percent of the rural population. More than 50 percent of residential water supplies in eastern Kentucky rely on ground water. If aquifers are protected from pollution by wellhead protection programs and old wells are retrofitted to prevent direct contamination, then ground water will continue to provide a reliable water supply in many rural areas of the basin. However, for most of the basin, few wells will have yields adequate to supply a large demand. Ground water from present wells will not provide an adequate supply for communities with a population of over a few thousand. Limited discharge data available for springs and large wells in the basin strongly suggest that the potential for ground water to supplement current supplies should not be ignored. Discharge from well fields and springs could be used to augment surface supplies during drought. A better understanding of the distribution and quality of ground-water resources is crucial for the citizens of the basin to fully benefit from ground water

    RiboTag : Ribosomal tagging strategy to analyze cell type specific mRNA expression in vivo

    Ribosome tagging has become a very useful in vivo approach for analyzing gene expression and mRNA translation in specific cell types that are difficult and time consuming to isolate by conventional methods. The approach is based on selectively expressing an HA-tagged ribosomal protein in a target cell type and then using antibodies against HA to purify the polysomes and associated mRNAs from the target cell. The original approach makes use of a mouse line (RiboTag) harboring a modified allele of Rpl22 (Rpl22-HA) that is induced by the action of Cre recombinase. The Rpl22-HA gene can also be introduced into the animal by stereotaxic injections of an AAV-DIO-Rpl22-HA that is then activated in Cre-expressing cells. Both methods for tagging ribosomes facilitate the immunoprecipitation of ribosome bound mRNAs and their analysis by qRT-PCR or RNA-Seq. This protocol will discuss the technical procedures and describe important considerations relevant to the analysis of the data
